BYU Health Study Survey

Community health assessment survey.

This survey is being conducted by BYU Public Health students who are interested in the health needs of Santaquin, Utah. For this survey you will be presented with health relevant questions and asked to identify opinions around them. Please be assured that your responses will be kept completely confidential and anonymous. The data collected will be used by the Utah County Department of Health to target specific health interventions within the community. 

The survey should take you around 5-10 minutes to complete. Your participation in this research is voluntary.
