Welcome to Santaquin City Passport Services! 

Our office provides assistance with all aspects of applying for a U.S. Passport. We can help you complete your application, review your documents, and provide on-site passport photos if needed. 

 

Office Hours: Tuesday–Thursday, 9:00 AM – 3:00 PM.  

Walk-ins cannot be accommodated.

How to Apply for a New Passport 

Follow these 4 steps: 

Step 1: Complete Form DS-11 

Fill out and print the form online: DS-11 Form 

Do not sign until you are instructed by a passport agent. 

 

Step 2: Bring Required Documents 

Bring proof of citizenship (original or certified birth certificate, naturalization certificate, or previous U.S. passport) and proof of identity (driver’s license, state-issued ID, or military ID), along with photocopies of the front and back of each. 

 

Step 3: Bring a Passport Photo 

One passport photo is required and must meet U.S. Department of State guidelines: Passport Photo Requirements 

Photos can be taken on-site for an additional fee. 

 

Step 4: Make Payments 

Applicants must make two separate payments: (1) U.S. Department of State and (2) Santaquin City:

*Optional Additional Fees: Expedited Service costs $60 and Overnight Delivery costs $22.05. 

Payment must be made by check or money order payable to “U.S. Department of State.”
Please write the applicant’s full name and date of birth clearly at the top of the check or money order.

You may write “Application Fee” in the memo line, but this is optional.

Renewals, Minors, and Passport Cards 

Renewals are available by mail or online for passports issued within the last 15 years if the applicant was age 16 or older at issuance: Renew a Passport 

Children under 16 must appear with both parents or legal guardians, and applicants aged 16–17 should appear with at least one parent or guardian. 

Passport Cards are available for frequent travel to Canada, Mexico, the Caribbean, or Bermuda by land or sea: Passport Book vs. Card
